The rule of hands to face changed a couple of years ago. In the trenches, you were often allowed to punch your defender inside the helmet (more like the facemask) when you took off the hand from the face as soon as contact was made. If you kept your hand on the facemask or helmet for more than one tick, it would be penalized. Also, if the blow from the punch to the helmet quickly snapped back the attacker's head, it was a foul.

 

The rule was amended in 2015 to basically "any contact to the helmet is a violation," it was flagged up to seven times per game during the preseason in the trenches, mostly at the line of attack.

 

I recall receiving a phone message from my brother following their first preseason match, during which multiple flags were placed against him for the rule and he was pissed. He was playing against a smaller defensive line and when he was about to punch his chest his hands just naturally rise up , and he makes a brief contact with the helmet of the defender. The incident was never previously reported and is now. We were both concerned about how it would fundamentally alter the play of offensive linemen.

 

As the regular season was upon us these calls vanished. I'm pretty certain Mitch was not cited for one of these in the course of the season. They were rarely strictly enforced according to the rulebook , and accidental contact to the helmet was ignored as being a violation madden 23 coins buy. A direct blow to the helmet was still enforced and play in the trenches didn't need to be fundamentally changed.
